Summary

A Flight Training Adelaide Diamond DA42 experienced a left engine failure on initial climb after take-off from Parafield. The instructor took control, feathered the left propeller, and returned for a circuit to a single-engine landing. Both the instructor and student were uninjured.

Investigation Findings (4)

  1. 01A blocked fuel injector in the Austro AE300 engine was identified as the cause of the engine failure.
  2. 02Blockage was caused by contaminant particles from a batch of Jet A-1 supplied at Parafield on the previous day.
  3. 03Fuel from the suspect batch was tested and found to contain particles at the borderline of the specification limit.
  4. 04Flight Training Adelaide implemented a secondary fuel filter screen in its refuelling system.
