Summary
A Piper Seneca V operated by Broken Hill Aviation Academy completed a gear-up landing at Broken Hill Airport during a training flight. The aircraft slid along the runway on its nacelles and propellers for approximately 280 m before coming to rest on the runway centreline. The instructor and student were uninjured.
Investigation Findings (4)
- 01The crew had selected gear up after a missed approach and did not select gear down before the subsequent landing.
- 02The gear warning horn was found to be intermittently faulty, meaning the crew received no aural warning.
- 03The pre-landing checklist was completed but neither crew member confirmed gear-down visually.
- 04Broken Hill Aviation Academy implemented a mandatory visual gear-down check as a discrete checklist item separate from the pre-landing flow.
